The person (adult or child) whose name was changed was born overseas.
If the person whose name was changed was born in Victoria, visit Get a birth certificate.
If the name was changed interstate, contact the Registry in that state or territory.

You may be able to prove your identity online. If not, you can still prove your identity by mailing certified copies to us.
One of the easiest ways to prove your identity is by presenting your driver licence (if it confirms your residential address) and your Passport.
Alternatively, there are other ways to prove your identity.
If you're applying for your own certificate, you only need to provide proof of your identity.
If you're applying for someone else's certificate, you also need to prove that you're authorised to do so. Read the ‘Get someone else’s certificate’ tab on this page.
Applying for a certificate on behalf of someone else is similar to applying for your own certificate. The only difference is that you need to prove you're authorised to do so.
You don't need authorisation if you're applying for:
You can apply for another adult's certificate if you have their authorisation.
This includes your own children if they're over 18 years of age. At 18, children are legally recognised as adults and access to their records is restricted.

We send legal certificates by express post, for security.
For commemorative certificates, you may only choose standard post. This is because they're not official identity documents. Legal and commemorative certificates will arrive separately.
