How to identify the cause of death forms.
Copies of the cause of death certificate
Medical practitioners need to leave a printed copy of the cause of death certificate (MCCD/MCCPD) with the body for funeral directors.
The Births, Deaths and Marriages Registration Act 1996 does not provide for doctors to charge for completing these notifications.
Funeral directors cannot access the cause of death from their accounts.
